I, the undersigned, do hereby voluntarily submit my accurately filled application for registration in the 2026 Wushu Taekwon-Do Academy's Winter Invitational Tournament. I agree to waive any claims against Chris Leyesa and any person or organizations connected with Wushu Taekwon-Do Academy Inc, and said tournaments for any injuries I may sustain. I also assume full responsibility for all my actions in connection with said tournament. I further agree that tournament directors, for publicity or promotion, can use any pictures taken of or by me in connection with said tournaments without compensation at this time or any other time.
